You are only entitled to an NHS Health Check from the local authority in which you live.
This service is available to eligible residents or patients registered to a GP in this catchment area.

Referral method

If you are 40-74 years old without a pre-existing condition and are registered with a GP you should receive an invite from your GP for a free NHS Health Check once every five years. You should be offered your first health check when you are 40 and again at age 45 50 55 60 65 and 70. Once you get your invitation follow the instructions to book your appointment.

If you think you are eligible for an NHS Health Check and you are in a milestone birthday year but you have not received an invite please contact your GP surgery to find out if they offer NHS Health Checks. Alternatively you can contact the Council's public health team by emailing phdesk@cornwall.gov.uk.

If your GP is unable to offer you an NHS Health Check, or do you not meet the eligibility criteria, there is still lots you can do to help reduce your risk of developing cardiovascular disease in the future. Making small changes to your lifestyle can have a big impact, including stopping smoking, eating a healthier diet and being more physically active If you are thinking about making lifestyle changes, Healthy Cornwall (www.healthycornwall.org.uk ) is available to support you. It is also a good idea to get your blood pressure checked regularly. Most community pharmacies offer free blood pressure checks to people aged 40 or over who do not already have high blood pressure. Alternatively, you can borrow a blood pressure machine from your local Cornwall Council library and check your blood pressure at home.

Description of service

The NHS Health Check is a simple check of your heart health. Aimed at adults in England aged 40 to 74, it checks your vascular or circulatory health and works out your risk of developing some of the most disabling – but preventable – illnesses. It is free of charge, including any follow-up tests or appointments.

Service type

In Cornwall most NHS Health Checks are delivered in GP surgeries. Occasionally NHS Health Check are also offered in workplaces or other community venues.
